HERE’S HOW APPOINTMENTS WORK
#1 – If you have had labs, bloodwork, or blood draws in the past 90 days, please email them over to [email protected] if you did not already include them when booking your appointment.
#2 – Please ensure that you have completed both the Informed Consent Form and the HIPAA Consent Form prior to starting your session. The forms can be found at the links below:

#3 – Please be sure that you have completed your Pre-Appointment Update prior to starting your session. The Pre-Appointment Update can be found on your Appointments page. It’s the set of 2 (two) arrows that form a rectangle to the right of your Appointment Summary. (See image below for more information).
#4 – When you have completed your appointment, please review your Doctor! You can submit a review by going to the Appointments page and clicking on the green star to the right of your Appointment Summary (see images below for more information).
